As a member of a school team at St. Theresa's, each student-athlete is required to sign an Athletic Contract.  The contract includes sections on Academics, School Spirit, and what it means to represent St. Theresa's as a Student Athlete.

Student Athletes are also required to complete the Acknowledgement of Student Insurance form, which will communicate to the school the athlete's level of insurance coverage.

Gym and Weight Room Use
The students have the opportunity to use the gym and weight room at the school.  Please read here and print off the medical form and the contract, complete and return to Mr. Laurin.
